[mirror-admin] "file has vanished" again

I've been seeing it for years (it has come up here from time to time).
When I switched my rsync job to only run when fullfilelist changed, I
saw a reduction in "file has vanished" messages, presumably because my
job was only running after the mirrors synced a set of updates.
However, I'm seeing "file has vanished" over half the time, and the same
files tend to show up repeatedly from rsync run to rsync run.

One thing: I typically only see this on fedora-enchilada, not
fedora-epel.
