I've tested http://www.sipmly.com/ but am not using it yet.  With a 
penny a minute, it's pretty cheap.  They offer a free testing account 
and the configurations for freeswitch and asterisk are in the settings 
menu that's available once you sign up for the free test.  Mine is still 
active 3 months after I signed up.  If nothing else, the configs are 
instructive. I'm running freeswitch, not asterisk.  If you're just 
looking for a switch, it's a bit more intuitive than asterisk.
