We just went through the process at work. The gist is that if you can still
easily access readers, or that title is available in a different format, or
even still available on VHS with access to readers, it is a copyright
infringement to make a copy of it.

The main exception is if you (your institution) owns the copyright, then you
can make a copy/change the format, etc.
