I think I may have found it.  I'm running Cenos 4.4 on that system. I 
ran the tests on a Centos 4.5 system and they worked as expected.  From 4.4:

rpm -qf /sbin/pidof
SysVinit-2.85-34.3

 From 4.5:

rpm -qf /sbin/pidof
SysVinit-2.85-34.4

I googled for problems with pidof but didn't see any that resembled this 
on but someone must have found it.
