Generally, when it appears to come from someone you know, either they 
are infected with a virus that is using their address book, or they are 
in an address book of a machine that is infected.

Note, spammers also will pull groups of emails from public lists like 
ALE and send emails to the group from one of the email addresses in the 
group.
