I can't help you with the lesser distros, :)  but you can set up 
multiple queues for a single printer in SuSE via Yast print setup.  I've 
got 5 queues for my color laser.  Simply go to print configuration and 
you should see an option to create a new queue for an existing printer.

It could be that the the version of xpdf and/or adobe you're using. 
They are likely not handling that first page properly when attempting to 
print it, thus you get the blank page.  I've seen this before on a rare 
occasion.
