This is a key statement.  They can't just perform a spacewalk under the 
shuttle.  They couldn't check the titles, and as such everything turned 
in to a [tragic] afterthought.

It's a shame; and I'm not justifying it, but it's a risky business for 
reasons exactly like this.  Every pioneer faces extreme risks...

It's a terrible loss, because those very pioneers are some of the most 
skilled/well-rounded Americans...
