Yes, it's the theme.<br>

Not all themes have the window icon in the window title bar. You'll
need to change themes to one that has that if you want it.<br>
<br>
None of the default themes I'm aware of for the major envrionments
do it anymore, with perhaps the exception of KDE, which might.
However, by changing the window manager's settings (or even the
window manager itself) you can get the window decoration behavior to
be anything you want it to be.<br>
