I use vncviewer -via sshmachine vncservermachine
I then get prompted for the ssh password unless keys are in place then the vnc password. All is tunnelled through the ssh connection. As long as the sshmachine knows the internal name (it can be localhost, too) it works very well.

Note: I only use Linux systems so this is easy. For a non-windows system you have to use ssh port forwarding. puTTy ona wndows machine makes for a mighty useful and capable vpn client when the office LAN is behind a Linux firewall.
Digressing ... I've never gotten it to work the other way. Using
"-L" works, but I don't think I've ever been successful in my
attempts to use "-R". Does anybody here use "-R" a lot?
